Volume 18, Number 5 ________________________________________February 2000

Message from the President

Greetings!

I first want to thank the efforts of Brain Z. and American IV for a very nice meeting. The weather put us back a week, but a good time was had by all. The presentation by Dr. McClain was captivating and informative. I would like to exten my thanks to Dr. McClain for fitting us into his busy schedule and commuting in the winter weather conditions. The "Y2K" discussion did not bring any horror stories out, but there were a few bugs.
I would like to bring to everyone's attention the Annual Election Meeting in May. Those members, who are interested in becoming an officer, please let us know what office you are interested in. The current officers are willing to remain in office, but our terms are up as of May 2000, Any member interested in being a part of the By-Laws committee, send me an email or catch me at a meeting. This committee will be limited to 12 members.
If you have any information you would like to share, remember we have two sources to use. Contact Terry Tracy for the Newsletter and Rich Richardson for the web site. See you in Febraury.

  2. Each report and its envelope shall be specifically identified, e.g. "User Facility Report" "Annual Report" etc.
  3. User facilities, importers, and manufacturers shall establish and maintain MDR event files.
  4. The front page of the FDA form 3500A is to be filled out by all reporters within 30 days (originally 10 days) of becoming aware of information that reasonably suggest that a device has or may have caused or contributed to a death or serious injury.
